Position Summary
The Health Services Practitioner will act as a subject matter expert in the home, providing education and clinician expertise in providing quality resident care. the registered nursing staff at Grandview Lodge while acting as a subject matter expert in the home. This position is responsible for planning, coordination and delivery of medical care to Grandview residents in collaboration with the leadership team and inter-professional health team. Accountable for their own practice, they will provide comprehensive health assessments, diagnose health/illness conditions, and treat and manage acute and chronic illnesses as a key part of the overall resident’s care plan.
Qualifications, Knowledge, and Skills
Education
- University degree (BScN – Registered Nurse currently registered and in good standing with the College of Nurses of Ontario)
- Ontario Primary HealthCare Nurse Practitioner Certificate Program
Experience
- Minimum 5 years related experience, preferably with at least two years in long term care (experience with the accreditation process, infection prevention and control, quality assurance, and risk management, would be assets)
Knowledge/Skills
- Sound technical knowledge relative to accountabilities to provide leadership and supervision from a clinical perspective.
- Excellent interpersonal skills including the ability to work effectively in a team environment and guide interdisciplinary team-based discussions to increase staff morale, team building, collaboration, and community pride, including managing conflict and emergencies using tact, diplomacy, empathy, and conflict resolution skills to resolve issues.
- Excellent verbal and written communication skills with the ability to communicate strategically and provide resolutions if necessary to handle significant contracts with long-term impacts.
- Demonstrated analytical and problem-solving skills involving occasional ingenuity, and refinement of procedures when necessary.
- Sound understanding of Accreditation processes and Required Organizational Practices.
- Good understanding of municipal government, its operations and services and legislation that affects the divisional responsibilities.
- A Valid Ontario Driver’s License and access to a reliable vehicle.
- Technology Aptitude
- Demonstrated intermediate computer skills to utilize various technological devices and programs while adhering to cybersecurity and best practices are followed.
- Intermediate knowledge of MS Office (Word, Excel, Outlook, PowerPoint)
- Computer proficiency in Point Click Care, or equivalent reporting software.
Responsibilities
The incumbent is responsible for, but not limited to:
Corporate
- Follow the GVL philosophy of delivering care to Residents by utilizing DementiAbility and GPA principles.
- Ensure the Resident’s Bill of Rights is respected and the guidelines set out by relevant provincial legislation, regulations, and directives are followed, by communicating with residents, visitors, and other personnel in a courteous manner.
- Collaborate with the Grandview’s leadership team, under the direction of the Administrator, and contribute to the development of goals and objectives for Grandview; chair or be an active participant in meetings designed to ensure actions are identified and taken contributing to continuous quality improvement at Grandview Lodge.
People
- Meet with residents, and/or families to resolve concerns related to the nursing department and medical care needs.
- Maintain regular communication with the Director of Nursing, Medical Director and Physicians.
- Act as a leader to educate staff on the nursing goals and objectives as they relate to ongoing charting and best practices for delivering care to residents.
Functional
- Nursing Operations
- Attend the home on a regular basis to provides comprehensive primary care including assessments for the Residents as part of the interdisciplinary health care team.
Complete daily clinical rounds. - Perform a person-centered health assessment of Resident on admission, annually and as needed (including Three month medication reviews).
- Advocate for and provide palliative and end of life care.
- Consult and collaborate with Medical Director/Physician when a diagnosis and or treatment plan is unclear or beyond the scope of a Nurse Practitioner.
- Perform admission and annual Resident examination and completes a written report of the findings of the examination.
- Order and/or perform appropriate screening and diagnostic investigation, interpreting results and assuming responsibility for follow-up. Diagnose acute and chronic health conditions.
- Manage the care of Resident by providing pharmacological, complementary and or counseling interventions and performs procedures with-in the NP scope of practice.
- Document clinical data, assessment finings, diagnosis, plans of care, therapeutic interventions, Resident responses and clinical rationale in a timely and accurate manner.
- Participate in Resident care conferences
- Increase continuity of care through collaboration, consultation and referral as appropriate with the health care team.
- Support and Advocate for the Residents best interests while engaging the Resident/Family/Substitute Decision Maker regarding care.
- Complete referrals to specialized consultants, services and other health providers.
- Conduct in-depth comprehensive health assessments integrating a wide range of complex information including: resident perspectives, staff perspectives, nursing and allied health theory and research.
- Act as an advocate through promotion of evidence-based primary health care services and nurse care of residents.
- Collaborate with the interdisciplinary team related to Resident plan of care.
- Maintain an awareness at all times of emergency procedures and participate in drills/exercises and associated educational opportunities.
- Attend the home on a regular basis to provides comprehensive primary care including assessments for the Residents as part of the interdisciplinary health care team.
- Legislative Compliance and Training
- Lead consultation and engagement with employees to foster employee commitment to a high level of performance in all areas of resident care, resident and employee safety and delivery of individual goals and objectives.
- Provide formal and informal training, guidance and coaching in the management of clinical care to the interdisciplinary team members, serving as a resource person
- Aid in the development, training and staff compliance with policies, public health standards, and accreditation standards through regular auditing, meetings, and problem solving.
- Participate in quality management activities and ensure activities carried out are in alignment with the home’s strategic plan and with a focus on continuously improving clinical care practices and resident care.
- Health and Safety
- Adhere to policies, procedures, protocols, routines and additional practices of the home, including infection control as required.
- Ensure the Resident’s comfort and safety through compliance with current Health and Safety Regulations and MLTC standards, as well as the application of appropriate Infection Control Principles and Practices.

Position Requirement(s)
- A current police check (LE 220 or Equivalent) from within the last 6 months
- Two-Step TB and Mantoux Test within the past 6 months
